How to Get Medical Records

Memorial Behavioral Health’s Medical Records staff is responsible to fulfill all medical record requests for medical information. 

It is our policy to charge a nominal fee for all copies except those sent from Memorial Behavioral Health to another social service agency or health care facility (doctor's office or hospital).

To request medical records be released, 

  1. Download a Consent for Release of Information form and complete in full. A return telephone number must be included in order to verify the request. If you would like to request a hard copy of Memorial Behavioral Health’s Consent for Release of Information form, please contact the site where you receive services.
  2. Sign the form. The patient who received care must sign the form. 
  3. Return the completed and signed form in person, by mail or fax to the Medical Records staff at the site where services are received.

If you elect to pick up your records, identification (driver's license or photo ID) will be required. Medical Records hours are Monday through Friday, 8:30 a.m. to 4:30 p.m. 

Most requests will be processed within two weeks. If a request is needed sooner than that, be sure to make note of that on the paperwork.
